西南地区HBV基因分型及其对阿德福韦酯治疗病毒学应答的影响

HBA genotypes in Southwest of China and their effect on virological response to Adefovir dipivoxil in treatment of CHB

目的调查西南地区HBV基因型的分布情况,观察该地区HBV基因型对阿德福韦酯(ADV)治疗CHB24w病毒学应答的影响。方法采用聚合酶链反应-限制性片段多态性分析(PCR-RFLP)方法进行基因型检测,对其中58例ADV抗病毒治疗者进行回顾性分析。结果545例HBV感染者,B型占58.9%(321例)、C型占40.9%(223例),B+E型0.2%(1例)。ADV治疗12w时,B型组和C型组病毒学应答率分别为68.6%和73.9%;两组HBV DNA中位数较基线分别下降2.4log10拷贝/ml和3.1 log10拷贝/ml,两组差异均无统计学意义。治疗24w时,两组病毒学应答率分别为85.7%和91.3%;HBV DNA中位数较基线分别下降2.7 log10拷贝/ml和4.4log10拷贝/ml,两组差异均无统计学意义。结论西南地区HBV基因型以B型、C型为主。基因型对ADV治疗的CHB病毒学应答无显著影响。

Objectire To investigate the distribution hepatitis B virus （HBV） genotypes in Southwest of China and observe the effect of HBV genotypes on virological response to Adefovir dipiroxil （ADV） in treatment of chronic hepatitis B. Methods Genotypes of HBV were assayed by PCRRFLP. 58 of cases with treatment of ADV were retrospectly reviewed for virological response. Resuits 545 of patients with HBV were identified,of which 321 of cases are genotype B and 223 of cases are genotype C. Thus,genotype B and C accounted for 58.9% and 40.9% respectively in this group of patients. Based on the more than 2 log10 copies/ml reduction and undetectable of HBV DNA level,there are no significantly difference for virological response rate （85.7% VS. 91.3% ,P〉 0.05）in 24 weeks between genotype B group and genotype C group among ADV-treated patients. The median reduction of serum HBV DNA at 24 weeks from baseline between two groups were similar （2.7 log10 copies/ml VS. 4.4 log10 copies/ml,P〉0.05）. Conclusions The prevalence of HBV genotype is genotype B and C in Southwest of China. The results suggested HBV genotype is not obvious retevant to the virological response of ADV-treated CHB during the 24 weeks.
