MMP-9、TIMP-1与TGF-β_1在妊娠期高血压疾病患者胎盘组织中的表达及意义 被引量:2

摘要 目的通过研究基质金属蛋白酶-9(MMP-9)及其抑制物(TIMP-1)与转化生长因子β1(TGF-β1)在妊娠期高血压疾病患者胎盘中的表达,探讨其与本病发病的关系。方法选择2005年11月—2006年11月在我院住院分娩的孕妇63例,分为子痫前期轻度组、子痫前期重度组、妊娠期高血压组、正常妊娠(对照)组,采用免疫组化法检测63例胎盘标本绒毛上皮组织中MMP-9、TIMP-1和TGF-β1的表达。结果①妊娠期高血压患者与正常妊娠者MMP-9的阳性表达,差异无显著性(P>0.05),而子痫前期轻度、重度组MMP-9阳性表达明显弱于正常妊娠组(P=0.012<0.05,P=0.0<0.01);子痫前期TIMP-1的阳性表达略弱于正常妊娠,但无显著性差异(P>0.05);MMP-9/TIMP-1明显下降。②子痫前期重度组胎盘TGF-β1含量明显高于对照组(P=0.019<0.05);子痫前期轻度组、妊娠期高血压组和对照组比较差异无显著性差异(P=0.215>0.05)。③对照组和妊娠期高血压组MMP-9和TGF-β1的表达无相关性(P>0.05);但子痫前期患者,MMP-9和TGF-β1存在明显完全负相关性(r=-1.0,P=0.0<0.01)。结论妊娠期高血压疾病患者MMP-9表达及下降,使得胎盘浅着床,胎盘局部缺血缺氧;TGF-β1表达增多,可能是引发妊娠期高血压疾病的重要原因之一。 OBJECTIVE To investigate expression of MMP-9, TIMP-1 and TGF-β1 in human of patients with hypertensive disorder complicating pregnancy for evaluating the role of them in hypertensive disorder complicating pregnancy. METHODS Immunohisochemical staining was used to examine expression of MMP-9, TIMP-1 and TGF-β1 in these samples. RESULTS (1) There is no significant difference on the positive expression of MMP-9 between normal pregnant women and gestational hypertensions (P〉0.05) . The positive expression of MMP-9 in the normal group was slightly higher than that in patients with pre-eclampsia (P=-0.012〈0.05, P=0.0〈0.01). The positive expression of TIMP-1 in the normal group was slightly higher than that in normal group, but there is no significance (P〉0.05) . And positive expression of MMP-9FFIMP-1 obviously decline. (2)Expression of TGF-β1 in the placenta of patients with severe preeclampsia was significantly higher than that in normal group (P=0.019〈0.05), but no significance among the patients with mild preeclampsia, gestational hypertensions and normal group (P=0.215〉0.05). (3)No correlation was found between MMP-9 and TGF-β1 expression in the normal and gestational hypertensions groups (P〉O.05), but MMP-9 and TGF-β1 expression showed completely negative correlation in the patients with pre-eclampsia (r=-1.0, P=-0.0〈0.01). CONCLUSIONS Lowed expression of MMP-9 and MMP-9FFIMP- 1 in syncytiotrophoblast might result in defective nidation and ischemia and hypoxia of partial tissue in the placenta. Increased expression of TGF-β1 might be one of the important factors leading to hypertensive disorder complicating pregnancy.
