纤维支气管镜在治疗呼吸衰竭中的应用价值

Value of bronchofibroscope in treating respiratory failure

目的探讨纤维支气管镜(纤支镜)在呼吸衰竭中治疗的价值。方法选择66例有痰液阻塞的呼吸衰竭患者,除常规吸痰,吸氧,抗感染,解痉平喘,化痰,无创机械通气等治疗外,其中33例应用纤支镜进行吸痰及局部药物治疗,为治疗组;其余33例为对照组,未进行纤支镜治疗。于治疗前后一周进行动脉血气分析。结果治疗组治疗前后一周动脉血气分析比较,差异有统计学意义;治疗组与对照组治疗一周后动脉血气分析差异有统计学意义。结论呼吸道感染及气道大量分泌物阻塞是造成呼吸衰竭的主要原因之一。通过纤支镜治疗可使气道内痰液得以清除,使缺氧及高碳酸血症得到迅速改善,是抢救重度呼吸衰竭的有效措施。纤支镜治疗方法在呼吸衰竭中的应用是一种有效、安全的技术。

Objective To study the value of bronchofibroscope in treating respiratory failure. Methods A total of 66 patients with respiratory failure and sputum blocking were treated by sputum aspiration, oxygen inhalation,antiinfection,bronehodilator and mechanical ventilation. Bronehofibroseope was used in 33 patients （treatment group） and others were not served as control. Arterial blood gas analysis was assayed one week before and after treatment. Results There was significant difference in blood gases in treatment group before and after treatment, so was between the two groups after one week＇ s treatment. Conclusion Respiratory infection associated with copious secretions in the respiratory tract is one of important factors that lead to respiratory failure. The sputum can be cleared out with bronehofibroseope and hypereapnia caused by hypoxia can be rapidly ameliorated. Bronehofibroseope is an effective and safe technique for treating respiratory failure.
