血管新生与卵巢浆液性上皮性肿瘤

摘要 目的探讨血管新生与卵巢浆液性上皮性肿瘤的进展与预后的关系。方法采用SP免疫组织化学染色法检测19例卵巢浆液性囊腺癌、15例交界性浆液性囊腺瘤和13例浆液性囊腺瘤中VEGF表达、MVD计数和PCNA的表达情况。结果浆液性囊腺癌中VEGF、MVD、PCNA的表达显著高于交界性浆液性囊腺瘤和浆液性囊腺瘤,交界性囊腺瘤中的MVD、PCNA显著高于浆液性囊腺瘤(均P<0.01);Ⅲ级浆液性囊腺癌中的VEGF、MVD、PCNA表达显著高于Ⅰ~Ⅱ级浆液性囊腺癌(均P<0.05);不同的临床分期之间VEGF、MVD、PCNA的表达没有显著性差异;VEGF、MVD、PCNA表达与临床预后之间没有显著性差异;VEGF、MVD表达与PCNA呈显著正相关(P<0.01);VEGF、MVD呈正相关(P<0.01)。结论VEGF、MVD和PCNA参与在卵巢浆液性上皮性肿瘤的发生、发展且与肿瘤的组织学分级相关,显示其可能成为评估卵巢浆液性上皮性肿瘤良、恶性的一个辅助指标。VEGF、MVD和PCNA与临床分期及预后无关。 Objective To investigate the expression of the vascular endothelial growth factor, microvascular density and proliferating cell nuclear antigen in the serous epithelial tumors of ovary and their role in the progression of the tumor. Methods The expression of the VEGF,MVD(indirecfly by CD31) and PCNA were detected by the routine SP immunohistochemistry in 19 cases of serous ovarian cystoadenocarcinomas, 15 borderline serous cystoadenomas and 13 serous ovarian cystoadenomas. Results The expression of the VEGF, MVD and PCNA in serous ovarian cystoadenocarcinomas were significantly higher than those in borderline serous cystoadenomas and serous ovarian cystoadenomas (P〈0.01 for all). The expression of the VEGF, MVD and PCNA in serous ovarian cystoadenocarcinomas grade Ⅲ were significantly higher than those in carcinomas grade Ⅰ~Ⅱ (p〈0.05 for all). There were no relationships between the expression of VEGF, MVD or PCNA and the stage of the carcinoma (P〉 0.05 for all). Correlation analysis showed that there were positive correlations between the expression of PCNA,MVD and VEGF. Conclusion Our findings in this study indicate that higher expression of VEGF, MVD and PCVA in serous ovarian cystoadenocarcinomas and borderline serous cystoadenomas may play a role in the progression of the serous ovarian tumors, and VEGF, MVD and PCNA may be the prospective auxiliary markers in the evaluation of the prognosis due to their higher expression in the higher grade tumors, although no relationship was found between the stage of carcinoma and their expressions.
