社区护理干预对中年高血压患者自我管理的影响

Influence of Community Nursing Intervention on Self-management for Middle-aged Patients with Hypertension

目的探讨社区护理干预对中年高血压患者自我管理能力的影响。方法将120例经住院治疗、出院后符合要求的中年高血压患者随机分为实验组和对照组,每组60例。实验组由专业护士以定期家访和电话访问的形式进行社区护理干预,包括为病人及家属提供心理、社会、服药、运动、饮食、血压自我监测等护理干预,持续时间为6个月。比较2组病人自我管理能力、血压、血脂控制情况。结果2组病人在提高自我管理能力、血压、血脂控制等方面有显著性差异(p<0.05或p<0.01)。结论社区护理干预能提高中年高血压患者的自我管理能力,对出院后康复及提高生活质量有积极作用。

Objective： To explore the influence of community nursing intervention on self-management for middle-aged patients with hypertension. Methods： 120 middle-aged patients with hypertension after being discharged were randomly divided into 2 groups, 60 cases in each group. The experiment group were given community-based nursing intervention through regular home visit or telephone interview by specific nurses for 6 months. The measures included providing psychological nursing, social support, medication, exercises, and diet instruct, and instruction for self-inspect on blood pressure. The self-management capacity and the condition of blood pressure, blood fat were compared between the 2 groups. Results： There were significant differences in the self-management capacity （p〈0.05） and the condition of blood pressure and blood fat （p〈0.01）. Conclusion： Community nursing intervention can effectively enhance the self-management capacity of middle-aged patients with hypertension, and play an important role in recovery after their being discharged and improving their quality of life.
