超声弹性成像鉴别诊断乳腺小实性病灶的价值 被引量:49

The value of ultrasonic elastography in differential diagnosing breast small masses

摘要 目的:探讨超声弹性成像在乳腺小病灶(最大直径≤10mm)鉴别诊断中的价值。方法:对经手术病理证实的直径≤10mm的142个乳腺病灶(Ⅰ组)和直径〉10mm的421个乳腺病灶(Ⅱ组)进行超声弹性成像检查,采用弹性成像5分评分法进行评价,弹性成像评分4分及以上为恶性,3分及以下为良性。以手术病理结果为诊断金标准,计算超声弹性成像诊断乳腺良恶性病变的敏感性、特异性和准确性。结果:Ⅰ组超声弹性成像诊断的敏感性、特异性和准确性分别为90.9%、99.2%、98.6%,Ⅱ组超声弹性成像诊断的敏感性、特异性和准确性分别为82.5%、97.6%、93.1%。结论:超声弹性成像有助于乳腺小病灶良恶性的鉴别诊断。 Objective To investigate the value of ultrasonic elastography (UE) in differential diagnosing breast small masses(diameter≤10 ram). Methods One hundred and fourty-two small breast lesions(diameter≤10mm, group Ⅰ ) and 421 large breast lesions (diameter 〉10mm, group Ⅱ) were examined by UE. All patients were confirmed pathologically. The lesions were evaluated by elastographic strain scores, which were classified into five grades. The lesions with score 4, 5 were diagnosed as malignancy,and the lesions with score less than 3 (including 3) were diagnosed as benignancy. Pathology was as golden standard for diagnosis, and the sensitivity, specifity and accuracy of UE for diagnosing malignant breast lesions were calculated. Results The sensitivity, specifity, and the accuracy of UE for diagnosing malignant breast lesions was 9(90. 9%, 99.2% and 98.6%in group Ⅰ and 82.5%, 97.6% and 93.1% in group Ⅱ, respectively. Conclusions UE is helpful in differentiating between malignant and benign breast small lesions.
