抚触合并非营养性吸吮对低体重早产儿生理性黄疸的影响

Effect of Caring Touch Combined with Non-nutritive Sucking on Physiological Jaundice in Preterm Newborns with Low Birth Weight

目的探讨抚触合并非营养性吸吮对低体重早产儿生理性黄疸的影响。方法将160例低体重早产儿根据不同护理方式分为4组,每组40例:单独胃管喂养(A组);胃管喂养合并非营养性吸吮(B组);胃管喂养合并抚触(C组);胃管喂养合并非营养性吸吮和抚触(D组),计算平均日排便次数、平均日排便量、胎粪排尽时间、黄疸持续时间、经皮胆红素(TCB)动态变化情况。结果D组患儿的胎粪排尽时间、黄疸持续时间和TCB均小于其他各组,D组与A组比较差异有统计学意义(P<0.05);平均日排便次数、平均日排便量D组多于其他各组,D组与A组比较差异有统计学意义(P<0.05)。结论非营养性吸吮和抚触可促进低体重早产儿胃肠道发育,减轻生理性黄疸程度。

Objective To explore the potential treatment effect of massaging and touching combined with non-nutritive sucking on the physiological jaundice in preterm newborns with low birth weight （LBW）. Methods One hundred and sixty preterm newborns with LBW（Jan 2003 to Oct 2005） were randomly divided into 4 groups according to the different feeding styles. Group A was fed by intermittent nasoduodenal or intermittent nasogastric feedings（ n =40）, Group B was fed by nasoduodenal or nasogastric feedings plus non-nutritive sucking （ n =40）, Group C was fed by nasoduodenal or nasogastric feedings plus massaging and touching（ n = 40）, and Group D was fed by nasoduodenal or nasogastric feedings plus non-nutritive sucking plus massaging and touching （n =40）. The daily fecesing times, daily fecesing quantity, fetus feces drainaging time, the duration of the jaundice, and the transcutaneous bilirubin（TCB） were observed dynamically. Results Group D had decreased the fetus feces drainaging time, duration of the jaundice and TCB than the other 3 groups, with significant difference seen between group D and group A （ P 〈0.05）. Group D had higher daily average fecesing frequency and daily average fecesing volume, with significant difference seen between group D and group A （ P 〈 0. 05）. Conclusion Non-nutritive sucking combined with massaging and touching can benefit the gastrointestinal development of preterm newborns with LDW and can help to alleviate the physiological jaundice.
