精神分裂症患者立体定向手术后远期认知能力的调查

Investigation on Long-term Cognitive Levels of Schizophrenia Patients after Stereotactic Neurosurgery

目的调查精神分裂症患者立体定向手术后的认知情况,为促进术后康复及回归社会提供依据。方法采用韦氏成人智力量表(WAIS-R)、韦氏记忆量表(WMS)、威氏康星卡片分类试验(WSCT)及简明精神症状评定量表(BPRS)、阴性症状量表(SANS),对887例患者进行手术前后的调查。结果(1)手术后1年,患者的认知指标均有明显改善;术后第2年,除言语IQ外,其他各指标较前1年有显著提高;第3、4年,除记忆指标较前1年有所下降外,其余各指标均稳定在较高水平。(2)手术后1年,患者的BPRS及SANS分值均显著降低,术后2～4年维持在较低水平。结论精神分裂症患者立体定向术后1～2年是认知水平的明显改善期,患者术后的远期临床效果稳定。

Objective To investigate the cognitive levels of schizophrenia patients after stereotactic neurosurgery, so as to provide evidence for post-operation rehabilitation and social re-adoption of patients. Methods Total 887 schizophrenia patients were surveyed with WAIS-R, WMS, WSCT, BPRS and SANS before and after operation. Results The cognitive level of patients was obviously improved 1 year after operation. Two years after operation, all parameters except for linguistic IQ were significantly improved compared with one year after operation. During the 3rd and 4th year after operation, the memory parameters were decreased compared with those of one year ago and all the other parameters were at a stable high level. The BPRS and SANS scores of patient significantly decreased one year after operation and remained at a lower level 2-4 years after operation. Conclusion The cognition level of schizophrenia patients is greatly improved 1-2 years after receiving stereotactic neurosurgery, and the long-term clinical effect is stable.
