经尿道和耻骨上穿刺激光治疗浅表性膀胱肿瘤临床观察

Transurethral and a suprapubic transvesical trocar surgery of superficial bladder tumors

应用Nd：YAG激光经尿道和耻骨上膀胱穿刺入路治疗浅表性膀胱肿瘤74例，获得满意疗效。随访63例，时间6～54个月，有7例肿瘤复发（11．1％）。认为经尿道治疗困难时，经耻骨上膀胱穿刺入路消除了经尿道治疗时的死角。在治疗前将丝裂霉素C注射于肿瘤基底周围粘膜下，对肿瘤基底充分照射和防止肿瘤复发有一定作用。该项技术汽化切除肿瘤快，几乎不出血，是一种安全、有效的治疗方法。

74 cases superficial bladder tumors were treated by transurethral and suprapubic transvesical trocar with Nd: YAG lasers, 63 cases have been followed up for 6~54 months and 7 cases (11. % ) were reccurrence. A suprapubic transvesical trocar approach is extremely important,since tumors at the bladder neck may be missed by the transurethral approach. The result suggested that submucosal injection at the base of the tumor with mitomycin-c before resection of bladder tumors might be an effectictive method for the prevention of tumor reccurrence. This technique are quick,minimal bleeding,less pain,safe and effective.
