兔颌面部感染伤口清创缝合的实验研究 被引量:2

摘要 目的:寻找感染伤口清创缝合规范合理的术式,为临床感染伤口清创缝合提供理论依据。方法:成年健康大白兔,于颌面部两侧及后大腿伤口接种金葡菌,选取已成功感染的兔20只。实验侧作特殊清创处理,对照侧作传统常规处理,观察伤口愈合情况。结果:实验侧:颌面部1期愈合19例,2期愈合1例;后大腿1期愈合12例,2期愈合8例。对照侧:颌面部1期愈合0例,2期愈合20例;后大腿1期愈合0例,2期愈合20例。结论:口腔颌面部感染伤口,通过特殊清创处理能达到1期愈合;肢体感染伤口同方法处理效果不及颌面部,但比常规处理效果好。以封闭式负压灌洗引流为主的特殊清创措施治疗感染伤口有重要作用。 Objective: To investigate a standard and suitable technique on debridement of maxillofacial infected wound and provide theoretical evidence on debridement of infected wound in clinic. Methods: 20 adult healthy Japanese rabbits were selected for this study and rabbits' bilaterally maxillofacial and two leg's wounds were inoculated with staphylococcus and infected successfully. Experimental sides were treated with special debridement and control sides with traditional one. The conditions of wound healing were observed respectively. Results.. Experimental sides showed that the first intention and second intention were 19 cases and 1 case respectively in maxillofacial areas; while the first intention and second intention were 12 cases and 8 cases respectively in legs. Control sides showed that the first intention and second intention were 0 and 20 case respectively in both maxillofacial areas and legs. Conclusion: The infected wound in maxillofacial areas can achieve the first intention with special debridement. The treating effect of the infected wound in maxillofacial areas is better than that in legs. The special debridement is superior to traditional one. The special debridement with hermetically negative pressure drainage plays an important role in the treatment of infected wound.
