增强型体外反搏治疗运动诱发心肌缺血、心绞痛的临床研究

摘要 目的 探讨增强型体外反搏(ECP)改善心肌缺血的作用机制.方法 选择稳定型劳力性心绞痛患者29例,在原有治疗不变的情况下,加用ECP治疗,30天为一疗程,每疗程前、后分别做平板运动试验.观察ECP治疗前、后下述指标的变化:①每周心绞痛发作的次数;②心率及心率和收缩压的乘积;③运动诱发心绞痛发作所需的时间;④运动后ST段下降0.1mv所需的时间;⑤运动持续的时间.结果 患者每周心绞痛发作的次数明显减少(P<0.05);心率及心率和收缩压乘积轻度变化(P>0.05);明显延长运动诱发心绞痛所需的时间、运动后ST段下降≥1mm所需的时间及运动持续的时间(P<0.05).结论 ECP治疗可以减轻冠状动脉疾病患者的心绞痛次数,延长其因运动诱发心肌缺血的运动时间. Objective To assess the function mechanism of external counter pulsation (ECP) during the course of improving myocardial ischemia. Methods Twenty -nine patients with stable tired anjina pectoris were admitteds, one therapeutic course was thirty days. Basic an- timyocardial ischemia drugs were taken and every patient was combined with ECP. The followings were used for analysis before and after one theraputic course : (1) the number of anginal attacks per week;(2) heart rate and heart rate times systolic blood pressure ; (3) time to onset of angi- na ; (4) time to 1 mm ST segment depresson; (5) exercise duration. Results The number of anginal attacks per week decreased remarbablely and there is little change in heart and heart rate times systolic blood pressure and the time to onset of angin and 1 mm ST segment depresson and exer- cise duration are all brolonged clearly( P 〈 0. 05 ). Conclusion ECP can control angian effectively and increase the exercise duration significently.
