摘要 心肌应变率及应变是组织多谱勒指标,不易受周围心肌的牵拉和心脏整体运动的影响,是评价节段心肌功能的新的量化指标。对正常心肌应变率与应变的规律性已有初步研究。缺血时应变率与应变下降,达峰值时间延后,结合多巴酚丁胺负荷试验对判断心肌梗死、心肌顿抑及心肌冬眠更有价值。应变率与应变可以灵敏反映心肌功能受损早期的舒张功能改变。观测原发性心肌病、先天性心脏病、糖尿病、尿毒症性心脏病的应变率与应变的变化,有助于早期诊断与观察疗效。并且为研究心肌激动顺序提供了新手段。 Myocardial strain rate and-straning is a index of tissue Doppler , which is not affected by drag from myocardium around and movemnt of the whole heart and is a new quantificat index in judging segmental myocardial tunction.There is a basic study about normal myocardial strain rate and its regularity Strain rate and straining can decrease during ischemia as well as its time of reaching the peak is postphoned, which has more value in diagnosing AMI,myocardial stunning and myocardial hibernation combined with dobutamine loading test.Strain rate and straining can reflect diastolic functional changes in early myocardial injury,of which the changes observed in primary cardiomyopathy,congenital heart disease,diabetic and uremic heart disease is helpful to its early diagnosis and effective observation, which also will provide a new method for the study of myocardial exciting sequence.
