胰腺类癌1例报告并文献复习

Report of A case of Pancreatic Carcinoid and Literature Review

目的:探讨胰腺类癌临床病理特点及治疗。方法:报告我院10 a间消化道类癌中1例胰腺类癌病例,结合国内外文献资料,对胰腺类癌的临床病理特点、诊断、鉴别诊断及治疗进行分析。结果:胰腺类癌极为少见,约占全部消化道内分泌肿瘤的1.4%。临床表现多样性,可有不典型类癌综合征表现。肿瘤细胞表达N SE,嗜铬素A(CgA),突触素及5-羟色胺等内分泌肿瘤细胞标志。胰腺类癌转移率高,5 a生存率低。结论:病理形态及免疫组化检查是胰腺类癌确诊的关键,治疗上应以根治性手术及彻底的淋巴结清扫为主。

Objective：To explore the clinic and pathological characteristics and treatment of pancreatic carcinoid. Methods ：The clinic data of one case of pancreatic carcinoid was analyzed and the relevant literatures was reviewed. Re- sults：Pancreatic carcinoid was a extremely rare tumor,made up 1, 4% of the total number of gastrointestinal earclnoid cases. The clinical presentations were various and part patients had atypical symptom of the carcinoid syndrome, The tumor cells could express neuron-specific enolase （NSE）, chromogranin A （CgA）,synaptophysin （SYN） and serotonin. They exhihitcd a high metastatic rate and extremely low five-year survival rate. Conclusion：The diagnosis of pancreatic carcinoid is mainly dependented on the pathologic way and the immunohistochemical markers. The patients should be managed by the radical operation and the elimination of relative lymph glands.
