支气管肺泡冲洗治疗老年性严重肺部感染疗效观察

Discuss effect of treat serious pulmonary infection by bronchoalveolar flush in older patient

目的探讨床旁纤维支气管镜吸痰及支气管肺泡冲洗对老年严重肺部感染的疗效。方法58例患者随机分为治疗组和对照组,治疗组常规纤维支气管镜检查后,反复冲洗,注入被稀释的敏感抗生素。结果治疗组治愈率50.00%,有效率92.86%;对照组治愈率33.33%,有效率76.67%;两组比较差异有统计学意义(P<0.05)。结论保持呼吸道通畅,消除呼吸道黏性分泌物,改善通气和换气是治疗肺部感染的关键。在老年性严重的肺部感染患者,若排痰无力,分泌物多,可考虑辅助纤维支气管镜肺泡冲洗治疗,以提高治愈率。

Objective To discuss effect of treat serious pulmonary infection by bronchoalveolar flush atter absorb off phlegm with fiberoptic bronchoscope in older patient. Methods The 58 patients were divided into two groups （treating group and collating group）. After routine examined with fiberoptic bronchoscope, the patients＇ bronchoalveolar were flushed again and again, and immited antibiotics that were diluted, in the treating group. Results In the treating group, the hysiced rate was 50.0%, the effectual rate was 92. 86%. In the other group, the hysiced rate was 33.33%, the effectual rate was 76.67%. It＇s the significance of statistics （P〈0.05）. Conclusions It is key to treating pulmory infection, that keeping expedite respiration, cleaning sticky secretions, bronchoaheolar lavage can increase cure, for older patients that have severe infection.
