摘要
目的探讨早发型重度子痫前期与晚发型重度子痫前期不同的临床特点及妊娠结局,同时分析早发型重度子痫前期患者,并发症的多少对围产儿的影响。方法对我院50例早发型重度子痫前期孕妇(A组)和60例晚发型重度子痫前期孕妇(B组)进行回顾性分析,A组发病孕周〈32周,B组发病孕周≥32周;将A组按照并发症的多少再分为A1和A2组,A1组并发症少于3个,而A2组的并发症大于等于3个。结果A组和B组FGR,新生儿窒息发生率和新生儿死亡率差异有统计学意义(P〈0.05—0.01),A组的并发症明显多于B组(P〈0.05—0.01),A1组与A2组比较,A2组FOR,新生儿窒息和围产儿死亡率明显高于A1组(P〈0.05—0.001),期待治疗的时间短于A1组。结论早发型重度子痫前期发病早,易伴发多种并发症,严重影响母儿的安全,期待治疗时要严密监测母儿的情况,适时终止妊娠是关键。
Objective : To explore the different clinical features and pregnancy results between the early - onset and late - onset severe preeclampsia, and analyze the effections of complications of the early - onset severe preeclampsia on perinates. Methods : Retro- spectively analyze 50 cases with early - onset severe preeclampsia ( called group A) and 60 cases with late - onset preeclampsia ( called group B), The group A with on - set 〈 32 weeks of gestation, and the group B with on - set ≥ 32 weeks of geatation, The patients in group A were devided into 2 groups called A1 and A2 according to the numbers of complications. The patients in group A1 were with less than 3 complications, and the patients in group A2 were with more than 3 complications. Results: The group A had statistical significance to the group B in the rate of newborn suffocation and newborn death ( P 〈 0. 05 - 0. 01 ). The complications of group A are more than group B ( P 〈0. 05 -0.01 ). The group A2 compared with group A1, the rate of FGR, newborn suffocation and newborn death is much higher than group A~ ( P 〈0. 05 -0. 001 ), the expectant management time is shorter than group A1. Conclusion Early - onset severe preeclampsia is easy to have many kinds of complications, affect maternity and perinates seriously. During the expectant management , the situation of maternity and perinates should be closely monitored and the preganancy termination be in good timing , and cesarean section in the first choice.
