摘要
目的:总结干细胞在妇科肿瘤领域的进展与应用前景。资料来源:应用计算机检索CNKI2003-01/2006-12与干细胞在妇科肿瘤方面应用相关的文章,检索词“干细胞;生物特性;妇科;恶性肿瘤;化疗”并限定文章语言种类为中文;同时检索Medline1995-01/2006-12相关文章,检索词:“,stem cell,gynecology,malignant tumor,chemotherapy”,限定文章语言种类为“English”。资料选择:对资料进行初审,选取与干细胞在妇科肿瘤领域的研究相关文献,无论是否随机、有无对照组均纳入,排除重复性研究。资料提炼:共收集到100余篇关于干细胞治疗妇科肿瘤的随机和未随机试验,28个试验符合纳入标准。资料综合:①造血干细胞辅助下的高剂量化疗药物治疗卵巢癌的疗效在大部分报告中得到肯定,但治疗方案选择与疗效目前尚无定论,鉴于干细胞治疗的可行性,必须进行前瞻性的随机试验以进一步明确疗效。②在宫颈上皮癌变过程中,干细胞的DNA异倍体率升高,DNA含量和DNA异倍体的剪除对早期识别细胞癌变及判断肿瘤患者的预后有重要意义。结论:干细胞具有在体内、外分化的高度分化潜能,在妇科肿瘤领域可以应用干细胞移植修复组织器官损伤,尤其是治疗一些由细胞死亡或功能失调所致的疾病,但由于技术不成熟,仍有许多问题需要解决。
OBJECTIVE: To summarize the progress and applied perspective of stem cells in the field of gynecological tumor. DATA SOURCES: The CNKI was undertaken to identify the relevant articles on stem cells in the field of gynecological tumor published from January 2003 to December 2006 with the key words of “stem cells, biological feature, gynecology, malignant tumor, chemotherapy” in Chinese. Simultaneously, the Medline was also searched for relevant articles published from January 1995 to December 2006 with the key words of “stem cells, gynecology, malignant tumor, chemotherapy” in English. STUDY SELECTION: The data were checked firstly, and articles on stem cells applying in gynecological tumor were selected. No matter articles with randomized trial or control group or not were included, but articles on repetitive study were excluded. DATA EXTRACTION: Totally over 100 articles on stem cells for gynecological tumor with randomized or non-randomized trial were collected, and 28 trials were accorded with the inclusive criterion. DATA SYNTHESIS: ①Curative effect of high-dose chemotherapeutics combined with haemopoietic stem cells on the treatment of ovarian cancer has been affirmed in most reports, but the choice and curative effect of therapeutic regimen have not been verified currently. As for the feasibility of cell therapy, prospective randomized trial should be conducted firstly to further identify the curative effect. ②During epithelial cervix canceration, DNA heteroploid rate of stem cells increases, and the DNA content and DNA heteroploid cutting are important for determining eadier canceration and judging prognosis in tumor patients. CONCLUSION: With the potential of high differentiation in vitro and in vivo, stem cells can be applied in repair of tissue and organ injury in the field of gynecological tumor, especially in the treatment of some diseases due to cell death or functional disorder. However, the technology of stem cell transplantation is not perfect, so many problems deserve further study.
