外固定架及克氏针治疗桡骨远端关节内粉碎骨折17例

Treatment of 17 Patients with Comminuted Intra-articular Fracture of Distal End of the Radius by External Fixation and Percutaneous Pins

目的:探讨桡骨远端关节内粉碎骨折的治疗方法。方法:本组17例均采用外固定架及克氏针固定。结果:术后X线检查,17例患者掌倾角8°～12°,尺偏角15°～25°,掌屈35°～90°,背伸25°～80°,前臂旋前60°～90°,旋后50°～90°。关节面分离移位压缩塌陷纠正≤2mm,功能恢复良好。结论:闭合复位或有限切开复位外固定架固定辅以克氏固定骨折或整复骨块,可以增加许多桡骨远端关节内粉碎骨折的稳定性从而获得满意结果。

Objective： To evaluate the treatment of comminuted intra-articular fracture of the distal end of the radius by external fixation and percutaneous pins . Methods： Seventeen intra-articular fracture of the distal end of the radius were treated with the external fixation and percutaneous pins . Results： Follow-up radiographs were all satisfactory. An average of 25° - 80°of extension , 35° - 90°of flexion , 60° - 90° of pronation , and 50° - 90°of supination were achieved . The accuracy of the reconstruction of the articular surface is important. All of the patients had a good or excellent result. Conclusions： Reduction with percutaneous pins and external fixation of comminuted intra-articular fracture can restore radiographic parameters to nearly normal values , maintain reduction throughout the period of fracture-healing , and pro- vide satisfactory functional results.
