儿童幽门螺杆菌感染与缺铁性贫血相关性探讨

Correlation of Helicobacter pylori (H.pylori) infection and iron deficiency anemia in children

目的探讨儿童幽门螺杆菌感染与缺铁性贫血(IDA)的相关性。方法以因慢性腹痛而行胃检查及Hp检测的69例患儿为观察对象,根据检查结果分为Hp阳性慢性胃炎组、Hp阴性慢性胃炎组及Hp阳性胃部镜下无损害组3组,所有病例均检测血红蛋白(Hb)、平均红细胞体积(MCV)、血清铁(SI)、血清铁蛋白(SF)、总铁结合力(TIBC)等IDA指标。比较三组之间的IDA指标和IDA伴发率。结果Hp阴性慢性胃炎组的IDA指标分别与Hp阳性慢性胃炎组、Hp阳性胃部镜下无损害组比较,差异均有统计学意义(P<0.01,P<0.05),而Hp阳性慢性胃炎组的IDA指标与Hp阳性胃部镜下无损害组之间比较,则差异无统计学意义(P>0.05)。Hp阴性慢性胃炎组的IDA伴发率均低于Hp阳性慢性胃炎组和Hp阳性胃部镜下无损害组(P<0.05),Hp阳性慢性胃炎组与Hp阳性胃部镜下无损害组之间的IDA伴发率比较差异无统计学意义(P>0.05)。结论儿童幽门螺杆菌感染与缺铁性贫血两者之间的相关性有统计学意义,且较慢性胃炎与缺铁性贫血的关系更为密切。

Objective To study the relationship between H. pylori （Hp） infection and iron deficiency anemia （IDA） in children. Methods Children who had undertaken gastroscopy and Hp test because of chronic stomachache were observed. According to the examination results, all the patients were divided into three groups. Group 1： Hp positive chronic gastritis group; group 2： Hp negative chronic gastritis group; group 3： Hp positive without gastric lesion group. IDA indexes such as hemoglobin （Hb）, mean corpuscular volume （MCA）, serum iron （SI）, serum ferritin （SF） and total iron-binding capacity （TIBC） were checked in all the groups. IDA indexes and related IDA incidence were compared among the three groups. Results After comparing IDA indexes between three gruops, it showed that there were obvious difference （P〈0.01, P〈0.05） between group 2 and group 1, group 2 and group 3, respectively. There was no obvious difference （P〉0.05） between group 1 and group 3. The incidence of IDA in group 2 was obvious lower （P〈0. 05） than that either in group 1 or group 2. There was no obvious difference （P〉0. 05） between group 1 and group 3. Conclusion There is obvious relationship between Hp infection and IDA in children and the correlation is closer than that of chronic gastritis and IDA.
