左侧结肠癌合并肠梗阻46例Ⅰ期切除吻合临床分析

Clinical analysis of one-stage resection and anastomosis of left colonic carcinoma and intestinal obstruction

目的探讨左侧结肠癌合并肠梗阻进行Ⅰ期切除吻合的临床效果。方法回顾性分析2000～2006年46例左侧结肠癌合并肠梗阻,术中结肠减压灌洗和术前应用从日本引进的经肛门肠梗阻导管进行Ⅰ期切除吻合的临床资料。结果本组病例Ⅰ期切除吻合成功,无吻合口瘘发生,治愈出院。结论左侧结肠癌合并肠梗阻经严格掌握适应证,完善围手术期处理,Ⅰ期手术切除吻合是完全可行的,值得推广。

Objective To explore curative effect of one - stage resection and anastomosis of left colonic carcinoma and intestinal obstruction. Methods The clinical data of 46 patients with left colonic carcinoma and intestinal obstruction from 2000 to 2006 who were treated with transanus ileus tube made in Japan before the operation and lavage decompression during the operation were retrospectively analyzed. Results No postoperative anastomotic leakage were found and all the patients were cured and discharged from hospital. Conclusions One - stage resection and anastomosis is feasible for patients who suffered from left colonic carcinoma and intestinal obstruction if we strictly control indications and improve peripheral disposition between the pre- and post -operation.
