HLA-A、B基因对儿童过敏性紫癜的遗传易感性

摘要 目的探索HLA-A、B等位基因对内蒙地区汉族儿童过敏性紫癜(AP)的遗传易感性。方法采用病例对照研究方法,引入PCR-SSO技术,在祖籍三代居住内蒙地区、无血缘关系、无与异族通婚史及其他免疫性疾病史和家族史的汉族人群中,选择儿童AP病例组50例和健康儿童对照组96例,作以HLA- A、B等位基因型别分析。基因频率比较在单因素四格表x^2或Fisher检验的基础上,作以多因素logistic回归分析。结果HLA-A※26、HLA-B※35和HLA-B※52基因频率分别是7%、11%和9%,分别与健康对照组的1%、4.7%和3.6%比较,差异有统计学意义(x^2分别是-、4.107和3.639,P分别是0.009、0.043和0.056)。经回归后,Wald分剐是4.783、4.205和4.293,P值分别是0.029、0.040和0.038,差异均有统计学意义(均P<0.05)。B分别是1.782、1.112和1.164,均B>0;OR分别是5.941、3.041和3.202,均OR>1,促进发病,其95%可信区间分别是1.203~29.331、1.050~8806和1.065~9627,其内均不包含1,与P意义相符,兼有统计学意义。EF分别是0.647、0.369和0.387,均EF>0。结论HLA-A※26、HLA-B※35和HLA -B※52等位基因可能是内蒙地区汉族儿童AP发病单体型中的3个遗传易感基因。 Objective To discuss the susceptibility of HLA-A, B allele to allergic purpura (AP) in children of Hans in Inner Mongolia. Methods 50 children with AP were taken as case group and 96 healthy children as control group in Hans, whose three generations resided in Inner Mongolia without consanguinity, history of mixed marriages, other medical history, and family history of immunity. Polymerase chain reaction sequence specific oligonueleotide probes technique was used to analyze the type of HLA-A, B allele. The comparison of gene frequency was made with logistic regression after x^2 or Fisher test. Results The gene frequencies of HLA-A※26 ( 7% ), HLA-B ※ 35 ( 11% } and HLA-B※52(9%) alleles in case group were significantly different as compared to that of control group ( 1% ,4.7% and 3.6% respectively) (x^2 = - ,4. 107 and 3. 639,and P =0.009,0.043 and 0.056 respectively). Through logistic regression,Wald values were 4.783,4. 025 and 4.293 ,and P were 0.029,0. 040 and 0.038 respectively. B values were 1. 782,1.112 and 1. 164 respectively, OR values were 5. 941,3. 041 and 3. 202 respectively, whose 95% confident intervals were 1. 203-29. 331,1. 050-8806 and 1. 065-9627 respectively,and which did not include 1 for all with statistical significance, EF were 0. 647,0.369 and 0. 387, EF 〉 0 for all. Conclusion HLA-A 26,HLA-B※35 and HLA-B※52 alleles maybe are the haplotype susceptible genes of AP of Hans in Inner Mongolia.
