Tei指数评价三尖瓣关闭不全时右心功能改变及其意义

摘要 目的探讨三尖瓣关闭不全对右心功能的影响及其意义。方法随机收集笔者所在医院2005年8月至2006年7月住院患者中多种原因所致不同程度三尖瓣关闭不全的连续病例50例(三尖瓣反流组),排除有明显心律不齐、中度以上心包积液、扩张性心肌病等相关心肌疾病患者。其中男性25例,女性25例,年龄35~71岁,平均年龄47岁(标准差16.5岁)。对照组20例,其中男性10例,女性10例,年龄32~70岁,平均年龄48岁(标准差15岁),排除冠心病、高血压心脏病及其他先天性心脏病等心脏疾患。超声检测三尖瓣反流程度、速度,估算跨三尖瓣压差及肺动脉收缩压,同时启用组织多普勒,测量、计算右心室Tei指数。结果与对照组相比,三尖瓣关闭不全患者组的右心室等容收缩时间及等容舒张时间之和IRT、ICT明显延长,ET明显缩短,右心室Tei指数明显升高(P<0.05)。结论三尖瓣关闭不全可导致右心功能减低,三尖瓣关闭不全程度越重,右心功能减低越明显。 Objective To study the effect of tricuspid insufficiency on right heart function and its significance. Methods A total of 50 patients with tricuspid insufficiency and 20 normal subjects were performed Doppler echocardiographic examination. According to the degree of tricuspid reflux and the speed of tricuspid insufficiency, the gradient pressure of right ventricule and pulmonary arterial systolic pressure were classified. Tel index was estimated by measuring with tissue Doppler. Results Compared with the normal subjects, the RV IRT and ICT were significantly prolonged and ET was significantly shortened in the patients with tricuspid insufficiency. A marked increase of Tei index was found in patients (P 〈 0.01 ). Conclusion It is demonstrated that tricuspid insufficiency could affect right heart function. The more severe intensity of tricuspid insufficiency occurs, the more decreased heart function is found.
