摘要
目的:探讨KAI1、MMP-9蛋白表达以及微血管密度(MVD)值与胃癌浸润转移的相关性。方法:在71例胃癌组织及20例正常胃黏膜组织中,应用免疫组织化学SP法检测KAI1、MMP-9的蛋白表达,用CD34标记新生血管内皮细胞并于显微镜下计数MVD,进而分析它们与胃癌临床病理特征间及彼此之间的关系。结果:胃癌中KAI1的表达明显降低,MMP-9的表达明显升高,MVD值在胃癌中也较正常胃黏膜中明显增高,两组间差异有显著意义(P<0.05);KAI1与MMP-9的表达均与浸润深度、淋巴结转移密切相关(P<0.05),此外,KAI1的表达还与TNM分期有关(P<0.05);MVD值与分化程度、淋巴结转移及TNM分期有关(P<0.05)。经统计分析KAI1表达阴性组MVD值显著高于阳性组,而MMP-9表达阳性组MVD值显著高于阴性组(P<0.05)。结论:KAI1、MMP-9及MVD值与胃癌的侵袭转移密切相关,可作为评价胃癌的进展与预后的有力指标;KAI1、MMP-9可能通过影响血管生成参与胃癌的进展过程,为临床针对胃癌的抗血管治疗提供新的思路。
Objective. To detect the expression of KAI1 and MMP-9 protein and microvascular density (MVD) in gastric carcinoma and explore their roles in the invasion and metastasis of gastric carcinoma. Methods: The expressions of KAI1 and MMP-9 were detected by immunohistochemistry in 71 specimens of gastric carcinoma tissues and 20 specimens of normal gastric mucosa. MVD was labeled by CD34. Results. The positive rates of KAI1 and MMP-9 in gastric carcinoma tissues were 36.6% and 67.6% respectively, and MVD was 33. 972±11. 706, the difference was significant between gastric carcinoma tissues and normal gastric mucosa cases. KAI1 and MMP-9 expressions were both correlated with depth of invasion and lymph node metastasis, and KAI1 expression was correlated with TNM stage too. MVD was correlated with differentiation, lymph node metastasis and the TNM stage. In addition,the mean MVD in KAI1 negative group was significantly higher than that in KAI1 positive group; and The mean MVD in MMP-9 positive group was significantly higher than that in MMP-9 negative group. Conclusion: The expression of KAI1 and MMP-9 and MVD significantly correlated with biological behaviors of gastric carcinoma, and KAI1 and MMP-9 may become prognostic markers and therapeutic targets in human gastric carcinoma.
