经皮激光椎间盘减压术治疗椎间盘源性腰痛 被引量:9

Percutaneous laser disc decompression in the treatment of discogenic back pain

摘要 目的:探讨经皮激光椎间盘减压术(PLDD)治疗椎间盘源性腰痛的疗效。方法:我院自2004年10月~2006年12月对56例经MRI检查及椎间盘造影确诊椎间盘源性腰痛患者,102个椎间隙,采用德国BIOTEC公司半导体激光仪治疗,激光功率12W,每个激光脉冲持续1s,间隔1s,照射能量600~1000J。疼痛视觉模拟评分系统(VAS)评分评价治疗效果。结果:手术时间15~60min,平均30min。56例随访1~26个月,平均10个月,49例有效(术后VAS评分改善≥3分41例,≥2分8例),7例无效,有效率87.5%(49/56)。结论:PLDD治疗椎间盘源性腰痛安全、有效、微创。[关键词]腰痛;椎间盘造影;椎间盘源性疼痛; Objective: To evaluate curative effects of Percutaneous laser disc decompression (PLDD) in the treatment of discogenic low back pain. Methods: A total of 56 patients with discogenic low back pain who were diagnosted by MRI and discography was included in the study. The PLDD was performed by using the BIOTEC semiconductor laser treatment, with a power of 12W, ls-pulseduration, ls-interval, and an energy of radiation of 600- 1000 J. Curative outcomes were evaluated based on the Visual Analog Scale (VAS) criteria. Results: The operation time was 15-60 min (mean, 30 min). Follow-up examinations were conducted in the 56 patients for 1 -26 months (mean, 10 months). The postoperative VAS scores were improved by ≥ 3 points in 41 patients and ≥ 2 points in 8 patients. Results revealed that 49 patients (87.5%) reported a good response and 7 (12.5%) reported no response. Conclusion: Percutaneous laser disc decompression is a safe, effective, and minimally invasive way that can be used for patients with discogenic low back pain.
