187例早期胃癌临床病理回顾性分析

A Clinicaopathological Retrospective Analysis of 187 Cases of Early Gastric Cancer

目的:总结分析早期胃癌的临床及病理特点,提高对早期胃癌的检出率。方法:选择开展内镜以来,20年间胃镜发现并行外科手术后经病理证实为早期胃癌的病例187例,对病理组织学特点及随访结果加以分析。结果:早期胃癌切除占胃癌切除的4.99%;电子内镜检查活检证实为早期胃癌的阳性率为93.1%(174/187);部位以胃窦居多占49.2%;Ⅰ型、Ⅱ型、Ⅲ型早期胃癌检出率分别为25.7%、54.0%及20.3%。结论:早期胃癌临床无特征性症状,诊断主要靠内镜医师认真细致观察和识别早期胃癌的形态改变。

Objective： To analyze clinical and pathologic features of the early gastric cancer. Methods：To study and analyse histopathologic and follow - up findings in 187 patients treated in our hospital in the past 20 years since the application of endoscopey, who after endoscopy and surgery have been pathologically identified as early gastric cancer. Results：Excision of early gastric cancer accounts for 4.99% of the total excision of gastric cancer. 174 cases of early gastric cancer are found out of 187 patients （93.1% ） by endoscopic biopsy, mainly in gastric antrum （49.2%）. The detectable rate of early gastric cancer type Ⅰ, Ⅱ, and Ⅲ is 25.7% ,54.0% ,20.3%, respectively. Conclusion：Early gastric cancer clinically has no characteristic manifestations and diagnosis mainly depends on careful observation through endcscopy and identification of its changes in pattern.
