螺旋CT联合肿瘤标志物CA19-9检测在诊断胰腺癌中的临床应用 被引量:1

摘要 目的:探讨CT扫描和血清肿瘤标志物癌抗原(CA19-9)检测联合应用对胰腺癌的临床诊断价值。方法:回顾性分析胰腺病变60例,全部病例手术前1周行螺旋CT增强扫描及采集外周血,采用化学发光免疫测定技术测定其血清中CA19-9含量,而后与临床病理结果进行对照研究。结果:胰腺癌的主要CT征象为局限性实性肿块,密度不均或呈低密度,增强扫描肿瘤的液化、坏死、低密度的改变更清晰,癌灶强化程度低于正常胰腺强化程度。CT、CA19-9对胰腺癌的诊断灵敏度为86.5%、82.7%,特异度均为62.5%、50.0%,符合率为83.3%和78.3%。CT、CA19-9联合应用时,灵敏度76.9%,特异度87.5%,符合率78.3%。结论:胰腺癌的CT表现有很高的检出率,如能结合CA19-9检测,在灵敏度变化不大时,明显提高特异度,具有较高的临床应用价值。
