老年2型糖尿病合并高尿酸血症的临床分析

Analysis of correlation factors of hyperuricemia with type 2 diabetes mellitus in elderly patients

目的探讨老年2型糖尿病高尿酸血症的临床特征。方法将1411例老年体检干部分为糖尿病高尿酸血症组、糖尿病正常尿酸血症组和非糖尿病对照组,测定其体重指数(BMI)、腰臀比(WHR)、血压(SBP、DBP)、血糖(FBG、PBG)、甘油三酯(TG)、胆固醇(CHO)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、空腹血清胰岛素(FINS)、血肌酐(Cr)、血尿酸(UA)。结果与老年糖尿病正常尿酸血症组相比,老年糖尿病高尿酸血症组的BMI、WHR、DBP、TG、FINS、HOMA-IR、Cr水平明显升高,合并代谢综合征、肥胖、高甘油三酯血症、低HDL-C血症和胰岛素抵抗的患病比例明显增高,而HDL-C水平降低。Logistic回归分析显示,DBP、Cr、TG、HDL-C、FINS、HOMA-IR是老年糖尿病高尿酸血症的危险因素。结论舒张压、甘油三酯、低密度脂蛋白、胰岛素抵抗、肌酐与老年2型糖尿病高尿酸血症密切相关。

Objective To explore the correlation factors of hyperuricemia with type 2 diabetes mellitus in elderly patients. Methods 1411 elderly cadres were included. Blood glucose （ FBG, PBG）, creatinin （Cr） , cholesterol （CHO） , triglyceride （TG） , blood pressure （ SBP, DBP） , body mass indes （BMI） ,waist hip ratio （WHR）,fasting serum insulin （FINS） and uric acid （UA） were measured. Resuits Eldly patients with hyperuficemia in type 2 diabetes had higher BMI, WHR, DBP, TG, FINS, HOMA-IR and Cr than those with normal serum uric acid. The hyperuricemia group had higher proportion of metablic syndrome and its related diseases, such as diabetes mellitus, obesity and insulin resistance than the normal-uricenmia group. Logistic regression analysis results suggested that DBP, TG, FINS, HOMA-IR and Cr were independent risk factors of hyperuricemia in eldly type 2 diabetes. Conclusions DBP, TG, FINS ,HOMA-IR and Cr were more related to hyperuricemia in eldly type 2 diabetes.
