联合应用前方牵引器关闭拔牙间隙并矫治恒牙早期安氏Ⅲ类错

Combined fixed appliance and protraction facemask to treat early permanent dentition Class Ⅲ malocclusion and close the extraction space.

目的:观察联合应用前方牵引器关闭拔牙间隙并矫治恒牙早期安氏III类错的临床疗效。方法:15例恒牙早期安氏III类错病例,上颌拔除两个第二前磨牙。上下颌粘结直丝弓矫治器,下颌全牙列平面塑料垫,排齐整平牙列后,联合应用前方牵引器关闭拔牙间隙并矫治恒牙早期安氏III类错。治疗前后分别摄X线头颅侧位片进行头影测量。结果:15例患者拔牙间隙关闭,安氏III类错纠正,SNA平均增加1.40,SNB平均减少2.50,ANB由-2.80增加到1.10。结论:在恒牙早期联合应用前方牵引器关闭拔牙间隙并矫治安氏III类错能促进上颌骨前移,抑制下颌骨向前方生长,可有效改善侧貌外形。

Objective： To investigate the clinical effects of combined fixed appliance and protraction facemask to treat early permanent dentition Class III malocclusion and close the extraction spaces of second premolars. Method： 15 cases of Class Ⅲ malocclusion were selected. The second premolars were extracted in maxilla. Fixed appliance were bonded in maxilla and mandible while occlusal plate on the mandible. After ligned and leveled, protraction facemask was used to correct Class III malocclusion and close the extracted spaces. Lateral cephalometric films were taken at beginning and the end of the treatment and compared. Result： 15 cases extracted spaces of Class Ⅲ malocclusion have been closed, while corrected Class III malocclusion. SNA increased 1.40 averagely, SNB decreased 2.50 averagely; ANB was increased from - 2.80 to 1.10. Conclusion：It suggested that early treatment used fixed appliance and protraction facemask in Class Ⅲ malocclusion may improve maxillary growth and inhibit mandibular growth, and while closed the extracted spaces.
