前列腺增生术后并发尿失禁的原因分析(附30例报告)

目的:探讨BPH手术治疗后并发尿失禁的原因和对策。方法:对用不同方式行前列腺摘除术后尿失禁30例患者的临床资料进行回顾性分析。结果:耻骨后前列腺摘除术后尿发生失禁12例,TURP与TUVP术后16例,改良Madigan术后2例。患者经行尿动力检查、排尿期尿道造影及尿道膀胱镜检查,明确发生尿失禁病因,并针对病因采取保守或手术治疗,尿失禁消失或改善。结论:选择正确摘除前列腺的手术方式对预防手术后尿失禁十分关键,术中保留完整的尿道外括约肌可以有效预防术后尿失禁的发生。

Objective.. To evaluate the cause and treatment for postprostatectomy urinary incontinence. Methods：The data of 30 cases with urinary incontinence after different approachs of prostatectomy in our hospital from 1989.1.-2005.12. were studied retrospectively. Results：Twelve cases of urinay incontinence occurred after retropubic prostatectomy, 16 cases after TURP and TUVP, 2 cases after improved Madigan procedure. Conclusions：It is important to choose appropriate means of prostatectomy and remain integrity of the urethral sphincter.
