摘要
应用超声心动图监测,经导管关闭先天性心脏间隔缺损9例。术中监测的主要内容有:①确定关闭器在心内的位置;②显示残余分流和瓣膜返流;③观察房间隔缺损球囊探测的过程。结果表明:超声心动图在显示心内结构、关闭器位置、残余分流和瓣膜返流等方面有其独特性,同时可结合声学造影或术中需要进行多方面的监测,并用于术前诊断和术后随访。提出超声心动图是经导管心脏间隔缺损关闭必不可少的一项监测手段,尤其对房间隔缺损的关闭术,可望不久的将来能在超声室进行。
This paper reports the monitoring of transcatheter closure of congenital heart septal defects by echocardiography in 9 patients.The principal monitorings conducted during the catheterization are as follows:(1) Finding the location of the occluder in the heart;(2) Displaying residual shunt and valvular regurgitation;(3) Observing the course of probing atrial septal defects with balloons.The results indicated that echocardiography played a unique role in displaying cardiac structure, the place of occluder, the residual shunt and valvular regurgitation.It can also be used for detection in many other ways, according to the ultrasonic contrast and the requirement of catheterization, and in the diagnosis before transcatheter occlusion and follow up. It was concluded that echocardiographic monitoring is essential to transcatheter occlusion of cardiac septal defects. Particularly in the future, the closure of atrial septal defect can be conducted in the ultrasound room.
