光化性扁平苔藓9例临床分析

Clinical and Pathological Analysis on 9 Cases with Actinic Lichen Planus

目的探讨光化性扁平苔藓临床及病理特征。方法回顾性分析本院诊断的9例光化性扁平苔藓,其中男8例,女1例,平均44.5岁。结果8例均可见双耳前及颞、颊部出现境界清楚的紫红或褐色斑片伴环状、网状融合的紫红色扁平丘疹,中央散在点状正常皮肤或色素沉着斑。双耳廓可同时受累,8例患者其他光暴露区域可见类似皮损。9例患者光敏测试均显示光敏感,组织病理特点除典型的扁平苔藓病理象外,还常见毛囊性扁平苔藓改变伴真皮浅层弹性纤维的嗜碱性变。结论光化性扁平苔藓在临床及病理上均有较特异的表现。

Objective To investigate the clinical and histopathological feature of Actinic Lichen Planus. Methods Nine eases of Actinic Lichen Planus were studied retrospectively. They were 8 males and 1 femal, the average age was 44, 5 years-old, Results The common skin lesions were purple or brown demarcated patches with annular and reticular confluent purple plane papules before the ears and on the temples and cheeks. Spotted normal skin and pigmentations were at the central of the lesion. Auricle and other light exposed areas were involved else. Photo sensitivity was detected in the all nine patients. Histopathology showed typical manifestations of Lichen Planus accompanied by feature of Lichen Planus Follicularis and basophilical degeneration of the upper dermal. Conclusion Actinic Lichen Planus was distinct dermatoses with the special clinical and histopathological feature.
