摘要
目的探讨胸水γ干扰素(interferon-γ,γ-IFN)、腺苷脱氨酶(adenosine deaminase,ADA)及β2微球蛋白(β2-microglobulin,β2MG)联合检测在结核性胸水鉴别诊断中的价值。方法γ-IFN检测采用酶联免疫吸附法(ELISA),ADA活力采用酶偶联速率法检测,β2MG采用免疫透射比浊法(ITA)检测;分别对40例结核性胸水和40例恶性胸水进引γ-IFN、ADA、β2MG含量检测。结果γ-IFN、ADA、β2MG在结核性胸水组中含量分别为(525.3±66.5)pg/mL、(63.8±20.5)IU/L、(4.05±0.85)mg/L明显高于恶性胸水组(65.2±36.5)pg/mL、(15.3±9.6)IU/L、(2.12±0.91)mg/L。结论联合检测γ-IFN、ADA、β2MG可显著提高结核性胸水鉴别诊断的灵敏度,具有重大临床参考价值。
[Objective] To explore the significance of Interferon-γ Adenosine Deaminase, and 152-microglobulin expression in pleural fluid and develop diagnostic application for tuberculous pleural effusion differentiation. [Methods] Detected expression of γ-IFN by ELISA analysis, ADA by enzyme kinetic analytical and 152-MG by im- munoturbidimetry assay (ITA), in 40 cases of tuberculous pleural effusion and 40 cases of malignant pleural effusion. [Results] The mean levels of γ-IFN, ADA and β2-MG in tuberculous effusions were (525.3±66.5) pg/mL, (63.8± 20.5)IU/L, (4.05±0.85)mg/L, significantly higher than that in malignant effusions which were (65.2±36.5)pμg/mL, (15.3±9.6)IU/L, (2.12±0.91)mg/L respectively. [Conclusion] Evaluation ofγ-IFN, ADA and β2MG in chest fluid can improve the differential diagnosis of tuberculous pleural effusion from malignant pleurat effusion and significantly increase the sensitivity of diagnosis.
