宫颈癌Ⅱ期术前介入治疗的临床价值

Clinical Value of Preoperative Interventional Therapy in Stage Ⅱ Cervical Cancer

目的探讨Ⅱ期宫颈癌患者术前介入化疗栓塞的临床价值。方法31例宫颈癌Ⅱ期患者，术前行超选择性插管化疗栓塞，10～14d后行宫颈癌根治术，分析其手术切除率、术中出血量及疗效，并与27例单纯手术病人进行对照。结果化疗栓塞组临床症状均有不同程度的缓解，术中平均出血量340mL，明显低于单纯手术组（560mL）；术中见瘤体有不同程度的坏死、缩小，分界变清，容易分离；病理示部分瘤细胞变性、坏死，瘤细胞数量减少，宫旁转移灶消失。术前化疗栓塞组3年和5年生存率分别为90．32％和75．86％，明显高于对照组（66．67％和46．15％）。结论子宫动脉化疗栓塞是Ⅱ期宫颈癌术前有效的辅助治疗手段。

Objective To study the clinical value in stage Ⅱ cervical cancer. Methods 31 patients with of preoperative interventional chemoembolization stage Ⅱ cervical cancer underwent preoperative superselective catheterization chemoembolization（embolism group） for 10～14 days, then radical resection of cervical cancer were performed. The operative rate, intraoperative bleeding and therapeutic effects were analyzed. 27 cases underwent simple operation（control group）. All the patients were followed-up for 5 years. Results The clinical symptoms of the embolism group were relieved to different extent, and during the operation, the mean blood loss of which was 340 mL, obviously lower than that of the control group, which was 560 mL. In the embolism group the tumor body was necrosis, diminution, tumor boundary become clear and easy to separate from the normal tissue. In post-operative pathological examination, some tumor cells were degenerate and necrosis, the cell population was reduced, and the metastasis near the tumor was disappearing. The 3-year and 5-year survival rates of the embolism group were 90.32% and 75.86% respectively, higher than that of the control group,which were 66.67% and 46.15% respectively. Conclusion Preoperative uterine artery chemoembolization is an effective adjunctive therapy in stage Ⅱ cervical cancer.
