经皮椎体成形术治疗中老年性椎体骨折所致疼痛的临床探讨 被引量:11

摘要 目的:观察经皮椎体成形术(PVP)治疗中老年性骨质疏松性多个胸、腰椎骨折所致疼痛的疗效,探讨其治疗方法和止痛机制。方法:以自固化磷酸钙人工骨为充填材料,在CT监视定位下,经单侧椎弓根穿刺行经皮椎体成形术治疗31例椎体压缩性骨折的患者。结果:31例52个压缩性骨折椎体只需治疗35个,PMMA平均充填量为2.6ml,术中PMMA渗出者3例3个椎体,无并发症。术后所有患者疼痛明显减轻或消失,总有效率100%,显效率为77.4%。结论:少充填量,单侧椎弓根穿刺单椎体的PVP治疗能有效地缓解骨质疏松性胸、腰椎压缩性骨折引起的疼痛。 Objective: To observe the effects of percutaneous vertebroplasty treatment on the pain caused by osteoporotic mulit-lumbar or thoracic vertebral compression fi-actures, and probe its treatment method and mechanism. Methods: 31 patients with osteoporotic compression fractures were treated by injecting PAMMA through the single pedicle puncture under the CT guidance. Results: Among 52 vertebrae of 31 cases, only 35 vertebrae needed to be treated. The average filling dosage was about 2.6ml. The leakage of PAMMA was observed in 3 vertebrae of 3 cases. The pain of all patients relieved or disappeared after operation, with 100% effective rate, and 77.4% excellent rate. Conclusions: Single pedicle puncture percutaneous vertebroplasty with minor filling dosage can effectively relieve the pain caused by osteoporotic compression fiactures.
