摘要
目的通过观察淫羊藿总黄酮(TFE)对超高分子量聚乙烯(UHMWPE)微粒刺激单核细胞(MOs)分泌IL-6的影响,探讨TFE抑制IL-6分泌的机制以及防治关节假体的无菌性松动的可能性。方法采集12例股骨颈骨折患者的外周血,分离MOs,分组培养。每例标本分成6组(n=12)。A组:仅MOs;B组:MOs+UHM-WPE微粒;C组:MOs+UHMWPE微粒+帕米膦酸钠(10 mg/L);D组:MOs+UHMWPE微粒+TFE(5 mg/L);E组:MOs+UHMWPE微粒+TFE(10 mg/L);F组:MOs+UHMWPE微粒+TFE(20 mg/L)。培养48 h后,ELISA法测定细胞上清液中IL-6的含量。结果A、C、D、E、F组上清液中IL-6含量分别明显低于B组(P<0.01);D、E组分别与C组之间的差异无统计学意义(P>0.05);F组明显低于C组(P<0.05)。D、E、F组两两之间差异有统计学意义(P<0.05)。结论UHMWPE微粒可以刺激单核细胞,使其分泌溶骨性因子IL-6的量增加。TFE与帕米膦酸钠作用效果相似,可以降低溶骨性细胞因子IL-6的分泌,随着TFE浓度的增加,其降低IL-6分泌量的作用加强。TFE有可能成为防治关节假体松动的药物之一。
Objective By observing the effect of total flavones of Epimedium (TFE) on the secretion of IL - 6 in human monocytes induced by ultra - high molecular weight polyethylene (UHMWPE) debris, to discuss the role of TFE in decreasing the secretion of IL - 6, providing a theoretical support for using the herb medicine to prevent and treat aseptic loosening of prosthesis. Methods Peripheral blood samples were obtained from 12 patients with fracture of the femoral neck. The monocytes (MOs) isolated from the blood samples were divided into 6 groups to carry out monocyte culture experiments with various compositions. A: containing MOs only; B: MOs + UHMWPE debris; C: MOs + UHMWPE debris + pamidronate (10 mg/L) ; D, E and F: MOs + UHMWPE debris + TFE (5,10 and 20 mg/L respectively). The mixtures were cultured for 48 hours to determine the concentration of IL - 6 by ELISA. Results The level of IL - 6 was much higher in group B than that in groups A, C, D, E and F (P 〈 0.01 ). There was no significant statistical difference between group C and groups D and E ( P 〉 0.05 ), but the differences were significant between group C and group F (P 〈 0.05 ), and especially, among group D, E, and F (P 〈 0.05). Conclusion HMWPE debris can stimulate mononuclear cells'secretion of IL -6. The inhibitory effect of TFE is similar to that of Pamidronate, and is proportional to the concentration of TFE. It appears that TFE can be a potential therapeutic agent for the prevention and treatment of aseptic loosening of joint prosthesis.
