喉癌45例远期疗效分析

Long-term Effect of Combined Treatment on Laryngeal Carcinoma Patients

目的探讨喉癌合理的治疗选择,提高临床疗效和病人生存质量。方法对我院1983年1月—2003年1月诊治的45例喉癌患者的的临床资料,远期治疗结果,进行回顾性分析。45例患者全部接受了外科手术治疗,其中行喉裂开声带切除术3例,垂直部分喉切除术29例,水平部分喉切除术6例,全喉切除术7例,15例做了根治性或功能性颈廓清术,术后均辅以放射治疗。结果随访3年以上,3、5年生存率分别为84.44%、77.78%,部分喉切除患者发音、呼吸和吞咽功能均得到恢复。拔管率100%。结论掌握好手术指征,充分了解肿瘤部位、侵犯范围和淋巴结转移情况,选择正确的术式,术后辅以放射等治疗,可望获得较好的治疗效果。

Objective To explore the proper treatment programme for laryngeal Carcinoma in order to guarantee the survival quality. Methods The clinical data of 45 patients with laryngeal carcinoma treated from January, 1983 to January, 2003 were reviewed and analyzed and their treatment results were summarized. All the patients were given operative treatment, i.e. three were given with laryngofission combined with cordectorny, 29 with vertical partial laryngeetomy, six with horizontal partial laryngectomy and seven with total laryngectomy. Of them, 15 ones were performed radical and functional neck clearance meanwhile. All the patients were administered postoperative radiotherapy. Results All the patients were followed up for more than three ycars. The 3-year arid 5 - year survival rates were 84.44% and 77.78% , respectively. Those who were treated with partial laryngeetomy rehabilitated their deglutitory and phonetic functions. The extubation rate was 100%. Conclusion The findings suggest that proper mastery of surgical indications, choice of laryngectomy way according to tumor location and lymph node metastasis, and postoperative radiotherapy act as the key to better therapeutic effect.
