十二指肠憩室合并上消化道大出血12例临床报道

Diagnosis and treatment of severe upper gastrointestinal hemorrhage complicated in duodenal divrticula.

目的 探讨十二指肠憩室合并上消化道大出血的诊治方法。方法 对12例十二指肠憩室合并上消化道大出血病人进行分析。结果 12例均为术后确诊，十二指肠横部憩室伴上消化道大出血5例，降部憩室伴出血7例，其中1例为第2次手术才确诊，有2例合并十二指肠球部溃疡，均治愈出院。结论 十二指肠憩室术前确诊较困难，术中探查尤为重要，探查时要特别注意出血是憩室引起还是伴发病所致。不同部位十二指肠憩室合并上消化道大出血，有不同的治疗方案。

Objective To investigate the dignosis and treatment of severe upper gastrointestinal hemorrhage complicated in duodenal diverticula. Methods Data of 12 patients with severe upper gastrointestinal hemorrhage complicated in duodenal diverticula were analysed. Results 12 patients were dignosed during operation and were Cured, Diverticula were located in the third part of the duodenum in 5 patients and in the second part in 7 patients, One patient was diagnosed during the second operation. Two patients complicated by ulcer in the duodenal bulb. Conclusion The diagnosis of hemorrhage complicated in duodenal diverticula is difficult before operation, We must differentiate bleeding caused by duodenal diverticula or comorbid during exploration. The treatment for the upper gastrointestinal hemorrhage complicated in duodenal diverticula depends on the location of hemorrhage.
