超重及肥胖者血浆游离脂肪酸与胰岛素抵抗的关系 被引量:10

摘要 目的观察超重及肥胖者血浆游离脂肪酸水平与胰岛素抵抗的关系。方法检测57例超重及肥胖者及56例体重正常者三酰甘油(TG)、空腹及口服葡萄糖后2 h之血浆游离脂肪酸浓度,行口服葡萄糖耐量试验(OGTT)及胰岛素释放试验,计算胰岛素敏感指数(ISI)。结果超重及肥胖组血浆游离脂肪酸(FFA)、血糖、三酰甘油(TG)、胰岛素(Ins)水平较对照组升高(P<0.01),FFA与三酰甘油(P<0.01)及血糖、胰岛素水平(P<0.05)呈显著正相关;胰岛素敏感指数(ISI)显著低于对照组(P<0.05),FFA与ISI呈显著负相关(r=-0.364,P<0.01)。结论超重及肥胖者血浆游离脂肪酸浓度升高是引起其产生胰岛素抵抗的重要因素。 Objective To investigate the relationship between serum free fatty acid(FFA) and insulin resist- ance(IR) in subjects with overweight and obesity . Methods The fasting and postprandial 2 hours concentration of serum FFA were measured, oral glucose tolerance test and insulin release test were performed in 57 overweight and o- besity subjects and 56 normal subjects matched for age and sex, calculating insulin sensitivity index(ISI) was calcu- lated. Results Overweight and obesity subjects had higher serum FFA, FBG, TG, Insulin compared with normal subjects(P 〈 0.01 ), there were significantly positive correlation between serum FFA and glucose, insulin (P 〈 0.05 ), TG( P 〈 0.01 ) ; ISI were lower( P 〈 0.05 ), There were significantly negative correlation between FFA and ISI (r = -0. 364,P 〈 0.01 ) . Conclusions The high concentration of serum FFA is the important factor of insulin resistance in overweight and obesity subjects.
