407例小儿EB病毒感染临床分析

The clinic characteristics of Eepstein-Barr virus infection in children (407 cases report)

目的:分析小儿EBV感染的临床表现特点及各种表现出现机率,提高对EBV感染的认识。方法:对407例经实验室确定为EBV感染的住院患儿,回顾性分析首发症状、病程经过和治疗转归。结果:EBV感染在小儿高发,男256例,女151例,男女之比为1.7∶1。EBV感染所致的典型表现占60%,非典型表现占40%。以单纯发热为首发症状就诊占40.29%,颈部包块和眼睑水肿为首发症状各占11.06%和16.21%。血常规白细胞增高占70.20%,淋巴细胞比例增高占74.20%。变异淋巴在发热的7～10天比例高达10%。肝肿大占50.86%,脾肿大占20.88%。肝功能异常经保肝治疗恢复快。更昔洛韦治疗效果较好。结论:EBV感染可累及机体多系统,主要表现有发热,咽痛、肝脾淋巴结肿大,眼睑水肿,鼻塞,皮疹,咳嗽。40%病儿临床表现不典型。

Objective： To analysis clinic characteristics of Epstein - Ban： virus （EBV） infection in 407 children. Methods： Four hundreds and seven eases diagnosised by EBV - DNA - PCR or EBV - VCA - IgM were studied retrospectively on main complain the courses of disease and the treatment. Results： Of 407 cases, 256 were male, 151 were female. The ratio of male to female was 1.7 to 1. Sixty percent of 407 cases demonstrated typical characteristics, 40% showed atypical features. For the first symptom and main complain, clinic characteristics showed 40. 29% cases with fever, 16.21% cases with eyelid dropsy, 11.06% cases with cervix block. Routine blood test showed 70. 20% cases with leukocyte increase and 74. 20% with lymphocyte increase. Ten percent cases were detected the different ion lymphocyte during course of 7 to 10 days of fever. Medical examination found the 50. 86% with hepatomegaly, 20.88% cases with spleen enlarged. Most cases were effectively treated by ganeiclovir. Conclusion： EBV infection in child could be involved all systems with divers clinic features. Recognizing the clinic characteristics would help clinician to diagnosis the disease.
