摘要 目的探讨甲状腺动脉栓塞治疗Graves病的解剖学基础。方法对22具(44侧)成人尸体甲状腺动脉进行解剖研究(尸体组),8例(16侧)甲状腺功能正常(正常组)及17例(34侧)Graves病患者(甲亢组)的甲状腺动脉造影进行影像学研究。结果从介入治疗的角度得到了甲状腺上、下动脉起点位置及方位,非腺体支与腺体血管内径、长度、角度等的解剖及影像统计学资料,并对腺体支的吻合和危险吻合进行了观察。结论①Graves病具有介入栓塞治疗的血供基础,其甲状腺动脉改变有利于介入栓塞治疗。自动塑形软头导管可超选择插入甲状腺动脉腺体支。甲状腺下动脉的袢状走行是识别它的重要影像学标志。栓塞应达到腺体小叶动脉,并避开非腺体支和危险吻合。②尸体组所测值可作为正常人甲状腺动脉的参数。 Objective To evaluate the anatomic basis of the thyroid artery embolization for Graves' disease. Methods Thyroid arteries were investigated, including 22 normal adult cadavers (cadaver group) ,8 non-thyropathy patients (normal group) and 17 hyperthyroidism patients (hyperthyroidism group). Results The anatomical and imaging data of the location, direction, inner diameters, length and angles of thyroid arteries were gained for the sake of interventional therapy. The anastomoses in the thyroid gland and dangerous anastomoses were explored. Conclusion ①There are interventional vessel basis in Graves' disease. The variety of thyroid arteries of Graves' disease is helpful for interventional therapy,and soft-tip auto-moulding catheter is easy for superselective catheterization inserted in gland branch artery of thyroid. The ansiform of inferior thyroid artery is an important imageological identification marker. Lobule arteries should be embolized, but the non-gland branch artery of thyroid and dangerous anastomoses should be avoided. ②The measurement data of thyroid arteries in cadaver group may be used as parameters for normal individual.
