二甲双胍对多囊卵巢综合征患者的影响 被引量:4

摘要 目的探讨二甲双胍治疗对多囊卵巢综合征(PCOS)患者是否有影响。方法选择PCOS患者26例作为PCOS组,给予二甲双胍治疗;同期选择月经周期正常的健康志愿者20例作为对照组。两组妇女于卵泡早期空腹12h后检测卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、胰岛素(INS)、生长激素(GH)、胰岛素样生长因子-1(IGF-1)。PCOS患者于二甲双胍治疗2个月后的卵泡早期复测上述各指标并与对照组进行比较。结果P-COS组二甲双胍治疗前血清LH、LH/FSH、T、INS高于对照组(P<0·05),GH低于对照组(P<0·05);治疗后除LH外,其他指标与对照组比较,差别均无显著性意义(P>0·05)。PCOS患者治疗前后LH/FSH、T、INS及IGH-1水平间差别均有显著性意义(P<0·05)。GH与BMI呈负相关关系(P<0·05);GH与T呈负相关关系(P<0·05)。PCOS患者二甲双胍治疗后有排卵组的LH、T较无排卵组低(P<0·05);有排卵组的GH较无排卵组高(P<0·05)。结论二甲双胍能降低血雄激素水平、减轻胰岛素抵抗,同时使血GH升高、IGF-1降低。 Objective To study the effects of metformin on patients with polycystic ovary syndrome (PCOS). Methods study group includes 26 cases of PCOS patients, 20 cases of healthy women were recruited as control group. Before and after the tratment with mefformin, FSH, LH, E2, INS, GH and IGF - 1 in blood of the two groups were detected during the early follicular phase of menstrual cycle. Results Before the tratment with mefformin, The level of LH, LH/FSH, T, INS was higher in study group than in control group ( P 〈 0.05 ) , but the level of GH was lower in study group than in control group ( P 〈0. 05). After the treatment with mefformin, the level of FSH, E2 , INS, GH and IGF - 1has no differences between the two groups. There were significant differences of the level of LH/FSH, T, INS and IGH - 1 In the study group before and after the treatment. GH was negatively correlated to BMI and T. in PCOS patients, the level of LH and T were lower in ovulating group than in anovulating group, but the level of GH was higher in ovulating group than in anovulating group. Conclusion metformin can reduce the level of androgenism and IGF - 1, alleviate insulin resistance and improve the level of GH.
