小剂量胺碘酮治疗老年室性心律失常QT间期与疗效关系研究

Correlation between efficacy of low-loading dose of amiodarone in treatment of ventricular arrythmias and QT interval in elderly patients

目的探讨小剂量胺碘酮治疗老年室性心律失常时QT间期与疗效的相关性。方法50例采用小剂量胺碘酮治疗的老年室性心律失常的患者,观察治疗4个月,患者室性心律失常QT间期延长与有效控制率的关系及其不良反应。结果50例老年室性心律失常患者治疗总有效率高达90%,且维持量小(140±37)mg/d,无明显不良反应,QT间期延长者较未延长者治疗效果更满意;QT间期未延长者延长负荷期后部分患者QT间期延长而取得满意疗效。结论小剂量胺碘酮治疗老年室性心律失常时QT间期延长者疗效更显满意。

[Objective] To evaluate correlation between the efficacy of a low-loading dose of Amiodarone and QT interval in the treatment of ventricular arrythmias in the elderly patients.[Methods] 50 patients with ventricular arrythmias were treated with a low-loading dose of Amiodarone for four months in the elderly patients.The therapeutic efficacy,QT interval and side-effects were observe.[Results] The total effective rate of elderly patients with ventricular arrythmias was 90%.The maintaining dose was（140±37）mg/d.No side-effects due to Amiodarone were observed.The therapy was more effective in patients with QT interval prolongation.If ventricular arrythmias was not controlled in patients with normal QT interval,QT interval was prolonged after additional loading dose of Amiodarone and ventricular arrythmias was controlled well.[Conclusion] There was obvious therapeutic efficacy of a low-loading dose of Amiodarone in the treatment of ventricular arrythmias.No severe side-effects were found.QT interval prolongation was very important for therapeutic efficacy in the elderly patients.
