半导体激光手术与喉部分切除术治疗早期声门癌疗效对比分析

Clinical evaluation on therapeutic effect of semi-conductor laser cordectomy and partial laryngotomy on early stage of vocal cord carcinoma

目的比较半导体激光声带切除术和喉部分切除术治疗早期声门癌的临床疗效。方法行激光声带肿瘤切除术(激光组)18例,部分喉切除术(手术组)20例,分析比较两组住院日、气管切开与否、术后喉功能保留情况及嗓音声学参数等指标以及随访结果。结果激光组术后住院日短,大多数可避免气管切开,喉功能保留良好,且部分患者可在局麻下完成手术。两组患者的嗓音声学参数差异具有统计学意义(P<0.05)。术后随访3～5年,两组术后复发情况比较差异无统计学意义(P>0.05)。结论激光治疗早期声门癌实现了微创治疗,综合评价效果明显优于喉部分切除术组。

Objective To investigate the therapeutic effect of semi-corductor laser cordectomy and partial laryngotomy on early stage of vocal cord carcinoma in a comparative way. Methods Included in this study was 18 cases treated by semi-conductor laser cordectomy （laser treating group, LTG） and 20 cases received partial laryngotomy （surgery treating group, STG）. A comparative analysis was made on their duration of hospitalization, tracheotomy received or not, the degree of laryngeal function reservation, the parameters of voice evaluation and the outcome of following-up after the operation. Results For the cases in the group of LTG, significantly superior therapeutic effect could be seen among them with much shortened hospitalization duration, most cases being avoided tracheotomy and much better laryngeal fimction reserved, besides a part of such cases operated on successfully under local anesthesia. The differences in the parameters of voice assays were very significant between the two groups following the operation （ P 〉 0.05）. Cordusions Laser cordectomy is a kind of minimal invasive surgery for the treatment of early stage of vocal cord carcinoma and it holds more significant therapeuticeffect than that of partial laryngotomy, when evaluated in a comparative way.
