摘要
目的利用31P磁共振波谱(magnetic resonance spectroscopy,MRS)分析肝细胞癌与正常肝组织的磷脂和能量代谢特点。方法16例正常人(对照组)及10例肝细胞癌患者(观察组)纳入本研究,对照组既往无肝病史,生化检查确认无异常,肝细胞癌患者经病理检查证实,用Sonata1.5T磁共振仪行31P-MRS检查,对磷酸单脂、磷酸单脂/β-三磷酸腺苷、磷酸单脂/磷酸二脂指标作独立样本t检验。结果对照组和观察组的上述3个指标均值分别为0.71±0.19、0.33±0.16、0.37±0.12和3.13±0.53、1.88±0.2、2.27±0.42,两组比较差异有统计学意义(P〈0.05)。结论31P-MRS能够无创性鉴别肝细胞癌与正常肝组织的代谢改变,可以对肝细胞癌进行早期诊断。
Objective To evaluate metabolism of different phospholipids and energy between hepatic cellular carcinoma and normal hepatic tissues by using ^31p magnetic resonance spectroscopy (^31 P-MRS) in vivo. Methods A total of 16 healthy adult persons with normal biochemical examination and without history of liver disease (control group) and 10 patients with hepatic cellular carcinoma (treatment group) were diagnosed with 31 P-MRS by using 1.5T MRI. Phosphomonoesterase (PME), phosphomonoesterase/β-ATP (PME/β-ATP) , and Phosphomonoesterase/phosphodiesterase (PME/PDE) were chosen for independent sample t test. Results The mean values of PME, PME/β-ATP, PME/PDE were 0.71 ±0.19, 0.33±0.16 and 0.37±0.12 respectively in control group, and 3.13± 0.53, 1.88 ± 0.2 and 2.27 ± 0.42 respectively in treatment group, with significant statistical difference ( P 〈 0.05). Conclusion 31 P-MRS can identify noninvasively the change of metabolism between the healthy adult persons and the patients with hepatic cellular carcinoma and provide early diagnosis of hepatic cellular carcinoma.
