抗炎症因子白细胞介素-13在原发性肾病综合征患儿血尿中的检测及其临床意义

摘要 目的探讨白细胞介素-13(IL-13)与原发性肾病综合征(PNS)发病机制的关系。方法采用液相蛋白芯片系统,对25例PNS患儿口服强的松治疗前后的血清、尿中的IL-13及20例对照组儿童血清、尿中的IL-13水平进行测定。并检测肾病组患儿治疗前的24小时尿蛋白定量、血清白蛋白、总胆固醇、甘油三酯。结果肾病组治疗前血清IL-13和相应对照组及治疗后比较无显著性差异(P〉0.05);肾病组治疗前尿IL-13和相应对照组比较有显著性升高(P〈0.05);治疗后尿IL-13和治疗前比较有显著性降低(P〈0.05);肾病组血清、尿IL-13分别与24小时尿蛋白定量、血清白蛋白、血清总胆固醇、甘油三酯无显著相关性(P〉0.05)。结论IL-13在PNS中可能不是特异性的抗炎症因子。尿IL-13水平升高可能因为病变的肾内固有细胞的分泌增多引起,是PNS病变中的局部性因子。 Objective Study the effect of IL-13 on the pathogenesis of childhood primary nephrotic syndrome. Methods Twenty healthy children were chosen as control group and Twenty-five children patients as patient group in our study. We adopted liquichip to measure the levels of IL-13 in patients, serum and urine before and after treatment with predisone. The levels of IL-13 in control group' s serum and urine were also measured. Meanwhile we examined the patients' 24 hours proteinuria, serum albumin, cholesterol and triglyceride before treatment .Results (1)Compared with control group, no significant differences were found in the level of IL-13 in patient's serum before and after treatment (P〉0.05). The level of serum IL-13 also showed no significant difference before and after treatment (P〉0.05). (2)The levels of IL -13 in the urine of the patients before treatment were significantly increased compared with that of control group and patients after treatment (P〈0.05).However, there were no significant differences in the levels of urinary IL-13 between patients after treatment and control group (P〉0.05). (3)The levels of IL -13 in serum and urine of the patients before treatment have no significant correlations to the levels of 24hrs proteinuria, serum albumin, cholesterol and triglyceride in the patients before treatment respectively (P〉0.05).Conclusions (1)IL -13 may not be a specific antiinflammation factor of PNS. Increasing levels of IL-13 in the urine of patients before treatment possibly resulted from excreting of inherence cells in the glomerulus. We think it is a partially-influenced factor in PNS.
