倾斜试验加舌下含服硝酸异山梨酯对血管迷走性晕厥诊断价值探讨

Diagnostic value of tilt table test with sublinguai isosorbide dinitrate or vasovagual syncope

目的探讨倾斜试验（TTT）加舌下含服硝酸异山梨酯（ID）对血管迷走性晕厥（VS）的临床诊断价值。方法对183例不明原因晕厥患者进行TTT加舌下含服ID与静脉滴注异丙肾上腺素（ISO）对比分析。并随机对183例不明原因晕厥的患者分为两组，静脉滴注ISO组103例，舌下含化ID组80例，选择无晕厥史39例设为对照1组、对照2组。结果ISO组与ID组的总阳性率、基础倾斜试验（BTTT）、药物激发后阳性率及各型所占比例比较差异无统计学意义（P〉0．05）。结论倾斜试验加舌下含服硝酸异山梨酯诊断血管迷走性晕厥，其敏感性、特异性高，无明显不良反应，是一种安全、简便、快速、可靠的诊断方法，值得推广应用。

To explore the diagnostic value of tilt table test with sublingual isosorbide dinitrate for vasovagual syncope. Methods 183 patients with syncope of unknown origin were selected, then carried out tilt table test（TIT）with them and compared using sublingual isosorbide dinitrate with using isoproterenol infusion. 183 patients with syncope of unknown origin were selected, 103 patients who were droped ISO through vine were named group ISO, 80 patients who sublingual administration ID were named group ID. 39 controls were selected who without syncope. Set them in two groups named group 1 and grope 2, Results Total positive rate , basic tilt test（ BTTT）, post medicine stimulate positive rate and percentage of different type were without obviously difference（ P 〉 0, 05 ） between group ISO and group ID. Conclusion Tilt Table Test using sublingual isosorbide dinitrate is more sensitive, specific and little obvious kickbacks to diagnose vasovagual syncope, and it is safe, convenient and reliable diagnostic method. It has some clinical diagnostic value. Worth generalization and application.
