摘要
目的探讨手腕部原发性腱鞘结核的临床治疗。方法1989年9月至2005年12月，对7倒手腕部原发性腱鞘结核患者采用手术完整切除病灶，全身抗结核药物和指导性复治等综合治疗，提出误诊原因及治疗要点。结果所有患者均一期愈合，术后随访10个月～10年，均未见复发。5例功能恢复良好，2例术前曾行局部激素封闭，导致粘连，术后遗有一定的功能障碍。结论本病早期缺乏典型的临床体征，极易误诊，需据临床表现和实验室检查进行分析，及时完整地切除病灶辅以全身抗结核治疗及功能锻炼，术后多能治愈。
To discuss the clinical treatment of primary tuberculosis in synovial sheath in hand and wrist. Methods From September 1989 to December 2005,7 cases of primary tuberculosis in synovial sheath in hand and wrist were treated using surgery and antitubercular agents and rehabilitation. Pointed out misdiagnosised reason and treatment points. Results All cases were cured. Excellent function was obtained in 5 cases. There were no recurrence after postoperative 10 months to 10 years follow - up. Conclusion There were lack of tipical signs for early diagnosis and easy to misdiagnosis. Diagnosis of primary tuberculosis in synovial sheath depends on the manifestation and lab test. Healing could be achieved after focal cleaning and antitubercular medication and functional rehabilitation were performed on time.
