经颈椎前路一体化颈椎钢板融合器治疗脊髓型颈椎病 被引量:4

摘要 目的探讨一体化颈椎钢板融合器(PCB)在治疗脊髓型颈椎病中的应用。方法采用经颈椎前路PCB固定治疗脊髓型颈椎病25例,根据症状改善情况、影像学资料及采用JOA评分系统评定患者神经功能恢复情况。结果25例随访6-20个月,所有患者术后未发生植骨坍塌,植骨挤出、压入椎管或椎间隙等植骨部位相关的并发症,供骨部位未发生血肿或感染,1例发生1枚螺钉松动。JOA评分:术前6-10分,平均8.7分;术后2周10-15分,平均14.8分;末次随访12-17分,平均15.7分;改善率为73.2%。参照Ch iles et al疗效评定标准:优6例,良15例,可3例,差1例。结论PCB可提供即刻的生物力学稳定性和良好的植骨融合环境。 Objective To evaluate the integrated anterior cervical plate cage benezech (PCB) implant for cervical spondylotic myelopathy. Methods The PCB implants were inserted in 25 patients. The preoperative and postoperative score of JOA were evaluated. Results All patients were followed up for 6 ~ 20 months. No intraoperative complica- tion occurred, and no complication was found in the donor site. One screw loosed in one case, 25 cases had the preoperative score of JOA for 6 -10 points (average 8.7 points) and the postoperative one for 10 - 15 points( average 14. 8 points) ,the last follow-up for 12 - 17 points (average 15, 7 points) , with mean recovery rate of 73.2%. According to the criteria of Chiles et al,the results were excellent in 6 cases,good in 15 ,fair in 3 ,and poor in 1. Conclusions The PCB implant can provide immediate biomechanical stability and allowing early functional exercises.
