摘要 目的;研究CT对新生儿缺氧缺血性脑病(DHIE)的检出率与羊水混浊度及Apgar评分(窒息程度)的关系。材料与方法:收集头位分娩时羊水混浊的患儿43例(其中窒息史的36例),均经CT扫描和Apgar评分。结果:1、DHIE的CT表现为脑水肿和颅内出血并按水肿范围分为轻,中,重三度。2、对羊水混浊的患儿,CT发现DHIE的阳性率为79.07%,其中在三度羊水混浊的患儿中,CT阳性率为86.96%。3、16例重度窒息儿的羊水均为三度混浊。结论:1、CT对DHIE的诊断具有重要价值。2、CT对DHIE的检出率与羊水混浊度及窒息程度是正相关系。3、对Apgar评分正常的窒息儿,CT诊断DHIE的阳性率仍高。 Purpose: To study the relation between CT detectable rate and amniotic opacity,Apgar evaluation when cephalic presentation in neonatal hypoxic ischemic encephalopathy (NHIE ). Meterials and Methods: Collected 43 new-borns of cephalic presentation with amniotic opacity (include 36 asphyctic cases). All applying CT scanning and Apgar evaluation. Results: 1、 The CTmanifestations of NHIE are encephaledema and intracranial hemorrhage. We grade three degrees according to rang of encephaledema. 2、 The positive rate of CT finding DHIE is 79.07% in new-borns with amniotic opacity. When the third degree amniotic opacity, the rate is 86. 96%. 3、16severe easphyctie cases are all the third degree amniotic opacity. Conclusion: 1、 CT as advanced methed should apply in diagnosis and cure of DHIE. 2、 The relation between CT detectable rate and degree of amniotic opacity, asphyxia are direct ratio. 3、 The asphyctic new-borns which Spgar-evaluation are normal have high CTdetectable rate to DHIE.
