改良法灌肠对于老年性菜花型低位直肠癌患者的效果评价

The Curative Effect Evaluation of Reforming Coloclysis Applied in Old Patienst with Fungating Type Low- set Rectal Cancer

目的 改进老年性菜花型低位直肠癌患者清洁灌肠方法，以减少不良反应、改善灌肠效果。方法 将62例老年性菜花型低位直肠癌患者随机分为改良组32例和常规组30例，常规组采用常规清洁灌肠方法，改良组采用改良灌肠措施实施（选择不同的灌肠卧位、肛管、肛管插入深度、灌肠液及灌肠量）。观察灌肠次数、每次灌肠量、灌肠液总量、灌肠液外溢情况、灌肠中的不良反应及洁肠效果。结果 与对照组比较，改良组灌肠次数、灌肠液量、灌肠总量及腹痛明显减少（P〈0．01）、无灌肠液外溢情况及肠黏膜损伤情况发生，洁肠效果明显优于对照组（P〈0．01）。结论 改进后的灌肠方法改善了老年性菜花型低位直肠癌患者的清洁灌肠效果，并减少了清洁灌肠过程中患者的不良反应。

Objective To decrease adverse effect , improve coloclysis effect with reforming coloclysis methods in old - fungating type low - set rectal cancer patients. Methods 62 cases with old - fungating type low - set rectal cancer was divided randomly into improvement group 32 cases and convention group 30 cases. Con- ventional cleaning enema was adopted in convention group, improved cleaning enema in the other group （ inclu- ding different enema clinostatism and tube, the different depth of the tube, the type and the volume of the colo- clysis liquid ）. Results Compared with control group , the times of enema, the volume of enema liquid and the frequency of abdominal pain decrease in treatment group （ P 〈0.01 ）, no cases with enema liquid flowing outside of intestines and intestinal mucosa damaged happened, the effect of cleaning ictestines better than that of control group. Conclusion The improved enema method can improve the effect of cleaning enema and decrease the adverse effect during enema in old - fungating type low - set rectal cancer patients.
