碘伏换药治疗糖尿病足溃疡的疗效观察

Efficacy evaluation of diabetic foot ulceration treated with iodophors dressings therapy

目的观察0.5%碘伏换药治疗糖尿病足溃疡的效果。方法将糖尿病足溃疡患者105例,随机分成两组,实验组55例,对照组50例。实验组:糖尿病足溃疡创面采用0.5%碘伏清洗,碘伏浸泡无菌纱布湿敷。对照组:采用常规方法,生理盐水100ml+0.5%甲硝唑100ml+庆大霉素16万U+山莨菪碱20mg+胰岛素24U浸泡无菌纱布湿敷。结果糖尿病足Ⅱ度溃疡的治愈率显著高于对照组(P<0.01),溃疡平均愈合时间明显短于对照组(P<0.01);Ⅰ度、Ⅲ度溃疡的治愈率,两组比较无明显差异(P>0.05),但Ⅰ度溃疡平均愈合时间明显短于对照组(P<0.01)。两组Ⅰ度、Ⅱ度、Ⅲ度治疗有效率比较无明显差异。结论0.5%碘伏换药治疗糖尿病足溃疡与常规方法相比Ⅰ、Ⅱ度溃疡愈合时间明显缩短,Ⅱ度溃疡治愈率明显提高,但两种方法对Ⅰ、Ⅲ度治愈率及各度溃疡治疗有效率无明显差异。

Objective To evaluate the efficacy of diabetic foot ulceration treated with 0.5% iodophors dressings therapy. Methods 105 cases of patients were randomly divided into the experimental group and the control group. 55 cases in the experimental group used iodophors dressings at the time of surgical debridement and changed twice a day thereafter. 50 cases in the control group received conventional hydropathic compress（arilin, gentamycin, 654-2 and insulin）continuously after surgical debridement. Results Healing rate of degree Ⅱ foot ulceration in the experimental group was higher than that in the control group（P〈0.01）and average healing time of degree Ⅱ foot ulceration in the experimental group was shorter than that in the control group（P〈0. 01）. There was not significantly different in the healing rate of degree Ⅰ and Ⅲ foot ulceration between the two groups（P〉0. 05） . Average healing time of degree Ⅰ foot ulceration in the experimental group was shorter than that of the control group（P〈0. 01）. The change of effective rate in degree Ⅰ , Ⅱ and Ⅲ foot ulceration was not significantly different （P〉0. 05）. Conclusions Iodophors dressings decreased healing time of degree Ⅰ and Ⅱ foot ulceration more effectively than conventional hydropathic compress and increase healing rate of degree Ⅱ foot ulceration.
